About Us

PRimary Home Health Care, Inc. is a local Registered Nurse owned and operated company.  Started in 2008, the provision of quality care to our patients and the mission of Promoting Healing and Wellness in the Home continue to be our only focus in serving our clients.

Colorado Springs and the surrounding areas are the only areas we service. This allows us to collaborate with our local hospitals, physicians, single entry points, and other medical facilities ensuring timely initiation and continuation of services.

We are licensed by the State of Colorado for both medical and non-medical care. Certification by Medicare and Medicaid ensures that we meet the needs of our community.  We continue to meet the gold standards of the Accreditation Commission for Health Care in home health since 2008.

Quality patient care in home health is our only business and we look forward to using our expertise to help meet the individual needs of your family.

Diana Prime
Co-Founder, President

Diana Prime, RN is the co-founder and President of Primary Home Health Care, Inc. which was established in 2008.  She is accountable for all facets of patient care, clinical, operational and financial leadership.

Prior to Primary Home Health Care, Diana Prime was a Chief Operating Officer for QS Nurses Home Care and Administrator for Western Home Care, a multi office agency across the front range of Colorado providing leadership for home health Medicare, Medicaid, and HMO programs.

Diana has served for eight years on the Board of Directors of the Home Care Association of Colorado representing up to 200 Colorado home health agencies in addition, was President of the Association from 2010-2012. As an advocate of home health, she also served on the Colorado State Medical Assistance and Services Advisory Council from 2005-2011.  Currently she remains active through the Association on several committees and through an appointment to the Colorado Dept of Public Health and Environment Home Health Informal Resolution Dispute Committee.

Colorado Springs continues to be home for Diana as it has been for 35+ years. Photography, snowshoeing, and paddle-boarding are a few of her personal interests.

Heather Shelton
Office Manager

Heather Shelton retired from the Air Force after 24 years in the medical service and started a completely new career here at Primary Home Health Care, Inc. Originally from Western New York, Heather has lived in various places along with certain other “vacations” courtesy of the military. 

As the Office Manager, she oversees the inter-workings of the office and conducts the billing for the Agency. She brought with her considerable knowledge from her previous career and has implemented various ways to reduce our carbon footprint along with reducing costs while not sacrificing our Mission. Heather takes pride in the fact that she has a part in caring for our Veteran population.

Heather resides with her husband and several animals that keep them busy. Some of her personal interests are reading, crocheting, baking and kayaking in the summertime along with snow bashing in the wintertime.
